Could this have been detected prior to finding out on site?

Drawing coordination either in 2 or 3 dimensions can avoid multiple trades items clashing, especially when you know what to look for.

For items such as pipe work there are certain ways you can only install it, to comply with building regulations. And likewise for structural items there is usually a maximum center they can be placed without contravening building regulations.
